A disease has a defined cause, such as a virus, bacteria, or specific genetic mutations, whereas a syndrome is characterized by a group of symptoms that often occur together but lack a proven cause, or may have many different causes.

While some cases of ALS can be linked to known genetic mutations, the majority of cases, which occur sporadically, have unknown causes. This could account for the wide range of factors observed in ALS, such as the rate of progression, survival time after diagnosis, and the different types of onset.
